OFFICE OF THE REGISTRAR

The Office of the Registrar is responsible for the management and release of the academic records of the students. Its decision carries weight in cases involving credentials, student records management, academic placement, and eligibility for graduation. Further, it also serves as the record keeper of the student's academic records from enrollment until graduation and after graduation. It maintains security, integrity and confidentiality. Visit the Office of the Registrar that is located just beside the entrance of Gate 2 of the main school building if you need to add or drop a class, file for graduation, request a transcript, change your major and/or add a minor subject.

In line with the College's Vision/Mission, the Registrar's Office commits itself to provide quality service to the students, faculty, and administration and other clientele.

OBJECTIVES

  1. To provide high quality professional services. To attend promptly in the issuance and release of requested records and documents. To show courtesy and professionalism in dealing with the clientele.
  2. To ensure the confidentiality and security of students' records.
  3. To ensure the accurateness of the students' records.
  4. To extend guidance and assistance to students with problems pertaining to academic regulations.
  5. To maintain harmonious and cooperative relations with all members of the community.

SERVICES OFFERED

A. REGISTRAR
  1. Assists the administration in the formulation and enforcement of policies on admission, enrollment, graduation and other related academic matters.
  2. She is a member of the Administrative Council.
  3. Responsible for:
    1. Admission of students
    2. Compliance of admission and academic requirements.
    3. Registration/Enrollment of students.
    4. Enforcement of school's regulation of transferees, academic load, subject sequence, cross-enrollment, graduation, changing/dropping/ withdrawal of enrollment and other related matters.
    5. Reviews/checks evaluation of scholastic records/credits for purposes of accreditation of transfer units, scholastic standing, promotion, graduation, etc.
    6. Commencement/Graduation.
    7. Assists in the selection of honor students by providing complete grades.
    8. Management, control, maintenance, accuracy, issuance/release of students' academic records.
    9. Orientation of students on office policies, rules and regulations.
    10. Information on statistical data of enrollment, graduates and dropouts.
    11. Makes Academic Calendar for the school year in coordination with the President and Department Chairpersons.
    12. Maintains linkages with other Registrars in connection with student records and registrars' concerns.
    13. Requests for F137E (for Basic Education and F137A and Original TOR of newly admitted college students).
    14. Applies for S.O. at CHED for student graduates.
    15. Submits readily required reports, surveys, and other data as requested by CHED.
    16. Cooperates with CHED by attendance to conferences, meetings and 0. seminars.
    17. Answers/Confirms education verifications of student graduates by different agencies.
    18. Performs other tasks as may be assigned by the President of the institution.
B. RECORDS IN-CHARGE: COLLEGE DEPARTMENT
  1. Accommodates, receives, processes and releases requests for transcript of records, certifications, transfer credentials, and other academic records.
  2. Keeps a daily record in all incoming and outgoing records.
  3. Maintains logbook for recording of incoming requests for academic records and their releases.
  4. Receives and disseminates communications to and from different departments.
  5. Attends to prompt compliance and release of requested records and other information.
  6. Attend the students and clients' queries.
  7. Classifies, arranges, files and stores current records and files.
  8. Classifies, arrange, files and stores inactive and graduated students' records
  9. Secures integrity and confidentiality of students' records.
  10. Prepares and issues transfer credentials/honorable dismissal to students who wish to transfer and graduates who applies for further studies.
  11. Receives and processes applications for Related Learning Experience of Nursing graduates.
  12. Files communications to their respective envelopes.
  13. Makes different forms available for students e.g. completion, evaluation sheets, application for graduation.
  14. Receives and records completion form of students
  15. Backs up files.
  16. Maintains cleanliness in the office.
  17. Prepares admission and enrollment paraphernalia.
  18. Coordinates admission and enrollment of new students.
  19. Reviews, checks, and verifies authenticity of submitted credentials of new students.
  20. Arrange and files credentials of new students.
  21. Informs the students of their course/subject deficiency. 22. Enrolls and issues load slip to students.
  22. Entertains dropping and adding of courses.
  23. Prepares enrollment list for CHED and offices.
  24. Provides class list to faculty members.
  25. Provides grade sheets to faculty members.
  26. Receives and records grades of students.
  27. Prints and issues grades of students.
  28. Issues application for graduation forms to graduating students.
  29. Assists in graduation activities.
  30. Prepares the program, diplomas, and certificates for graduation
  31. Helps the registrar in the evaluation of the graduating students
  32. Updates and separates individual files of graduates and transferees.
  33. Prepares the HEMIS (Higher Education Management information System) reports for CHED Regional Office.
  34. Prepares Inventory for every school year.
  35. Performs other duties as may be assigned by the Department Head.
